Your DNA, Your Health: What Precision Medicine Means for Women

A patient sits across from her physician with a question she has carried for years. Her mother had breast cancer at forty-eight. Her aunt had ovarian cancer. She has been told to keep an eye on it, but no one has ever walked her through what her own risk actually is, or what she could do with that information. She is not looking for reassurance. She is looking for a plan built around her.

That request, for care shaped by one person rather than a population average, is the idea behind precision medicine. The term sounds futuristic, but the principle is straightforward: your genes, your family history, your environment, and your own biology should inform how you are screened, what you are prescribed, and when you intervene. For a long time, medicine has worked the other way around, applying findings drawn largely from studies of men to bodies those studies rarely included.

What precision medicine actually means

Precision medicine, sometimes called personalized medicine, uses information about your individual makeup to guide prevention and treatment. Genetic testing is one of its most powerful tools, and it is not a single test. It is a category of tests that answer different questions:

  1. Hereditary risk testing looks for inherited changes that raise your lifetime risk of specific conditions, such as variants in the BRCA1 and BRCA2 genes or the genes associated with Lynch syndrome.

  2. Polygenic risk scoring estimates risk from the combined effect of many common genetic variations, which matters for the majority of people who do not carry a single high-risk mutation.

  3. Pharmacogenomic testing examines how your body processes medications, which can explain why a standard dose works well for one person and causes side effects in another.

  4. Early detection testing looks for signs of cancer already present in the body, sometimes before symptoms or standard screening would catch it.

Each of these turns a vague sense of risk into something a physician can act on. The value is not in the test result alone. It is in having someone who can interpret that result in the context of your whole health.

Why precision medicine matters more for women

The case for personalized healthcare is especially strong for women, because the conventional, one-size-fits-all model was largely calibrated to men. The evidence on this is not subtle.

A 2001 report from the U.S. General Accounting Office found that the majority of prescription drugs withdrawn from the U.S. market in the preceding years posed greater health risks for women than for men. That pattern has a biological explanation. Research published in Biology of Sex Differences concluded that prescribing equal drug doses to women and men neglects pharmacokinetic sex differences, risks overmedicating women, and contributes to adverse drug reactions that fall disproportionately on women. 

This is exactly the gap pharmacogenomic testing is built to close. A large analysis of the UK Biobank, the biggest study of its kind, found that 99.5 percent of participants carried at least one genetic variant relevant to how they respond to medication, and nearly 24 percent had already been prescribed a drug they were predicted to respond to atypically. Regulators have taken note. The FDA now identifies more than 200 medications whose labeling includes pharmacogenomic information, meaning your genetics can shape whether a given drug is the right choice and at what dose. 

A closer look at hereditary cancer risk

For the patient worried about her family history, hereditary risk testing offers some of the clearest answers in all of genomic medicine. According to the National Cancer Institute, more than 60 percent of women who inherit a harmful change in BRCA1 or BRCA2 will develop breast cancer during their lifetime, compared with about 13 percent of women in the general population. The ovarian cancer picture is similar: roughly 39 to 58 percent of women with a harmful BRCA1 variant and 13 to 29 percent with a harmful BRCA2 variant will develop ovarian cancer. Knowing this in advance changes the options on the table, from earlier and more frequent screening to risk-reducing strategies that simply are not offered to someone whose risk is assumed to be average. 

What genetic testing can and cannot tell you

Genetic information is powerful, and it has limits worth being honest about. A useful conversation with your physician separates the two:

  • A test can identify inherited variants that raise risk, but most variants shift probability rather than guarantee an outcome.

  • A test can flag medications likely to work poorly for you, but it does not replace clinical judgment about your full medication list.

  • A negative result for a specific gene does not mean zero risk, since lifestyle, environment, and genes not yet well understood still matter.

  • Results are most useful when paired with family history, which often reveals patterns a single test cannot.

Matching the right test to the right question

Personalized care is not about running every test on every patient. It is about choosing the tool that answers the question in front of you. The testing available through Ms.Medicine affiliate practices falls into two broad jobs: clarifying the risk you were born with, and catching disease early if it appears.

For understanding inherited and lifetime risk, a few tests do different work:

  • Hereditary cancer panels read a single blood sample for changes across dozens of genes tied to breast, ovarian, colorectal, and other cancers, including BRCA1, BRCA2, and the Lynch syndrome genes. Some panels go a step further. Myriad's myRisk, for example, pairs hereditary cancer screening with a personalized breast cancer risk estimate that is more accurate than traditional risk models used on their own, while Natera's Empower screens genes associated with more than a dozen cancer types. 

  • Polygenic risk scoring answers a question hereditary panels cannot. Fewer than ten percent of women referred for hereditary cancer testing are found to carry a harmful variant in a known susceptibility gene, which leaves most women without a clear picture of their risk. A saliva-based test such as geneType reads many small, common variations across the genome and combines them into a single risk estimate validated for the general population, useful precisely when the obvious genes come back normal.

For detecting cancer that may already be present, two newer blood tests extend what standard screening can do. Galleri, made by GRAIL, looks for a signal shared by more than 50 cancer types, including several that have no recommended screening test today. It is meant to be used alongside standard screening rather than in place of it, and it is currently under FDA review rather than fully approved. Shield, made by Guardant Health, took a different path: in 2024 it became the first blood test the FDA approved as a primary screening option for colorectal cancer in average-risk adults aged 45 and older, an option for people who will not complete a colonoscopy or stool-based test. A positive Shield result still calls for a colonoscopy to confirm. 

The point of offering several tests is not to overwhelm you with choices. It is to make sure the test you take is the one that fits your history, your age, and the question you actually need answered.
